Date of birth
Dates of service requested
Specific records requested (e.g., intake, progress notes, assessments)
Preferred delivery method (secure email, portal, mail)

3. Provide Identification if Requested
The provider may request a valid photo ID to verify identity.
4. Timeline for Response
o The provider must respond no later than 15 business days after receiving the request.
o If records cannot be produced within that timeframe, the provider must notify the consumer in writing.
5. Fees
o Providers may charge reasonable, cost-based fees for copies ($0.25 per page), but may not deny access due to inability to pay.
6. Denial of Access
o If access is denied, the consumer must receive a written explanation stating the reason and outlining appeal rights.
